Video Tape Masters Order Form

Thank you for licensing Dallas TeleLearning courseware. At the time of licensing, you may order your master set of tapes from Dallas TeleLearning. To order tapes, please fill out the form below and then click on the Submit Order Form button, or, if you prefer, mail in the printable version of this form. If you do not get a confirmation page, this form is not working properly and you should mail in the printable version.

Tape orders are accepted only for colleges and universities licensing Dallas TeleLearning courseware.

All tapes other than 1/2" VHS will have countdown, color bars, and tones unless otherwise specified.

NOTE: Client has six months after receipt of tapes to review tapes for defects.
